Outdoor work: the exposure that is not recreational and does not stop in September

Sun protection for people who work outside: cumulative daily exposure, the sites that take it, and why a workwear based strategy beats a sunscreen based one.

The short answer

Outdoor workers take the largest cumulative ultraviolet dose of any group, because the exposure is daily rather than occasional and runs across the whole season rather than a fortnight. The dose is also asymmetric: the face, the back of the neck, the forearms and, in many trades, one side of the body more than the other. Because the exposure is a work condition rather than a leisure choice, the strategies that work are the ones built into clothing and scheduling rather than the ones relying on remembering to reapply.

Almost every conversation about sun protection is a conversation about leisure. It assumes a beach, a holiday, a choice about whether to be outside. For people whose work is outdoors, none of those assumptions apply. Exposure is not a decision, it is a condition of employment, and it happens on the days with the highest ultraviolet as readily as on the days with the lowest, because roofs, roads, boats, farms and building sites do not observe the UV index.

This file is about that exposure. It is not medical advice and it is not employment advice. It is a description of the conditions and of the strategies that survive a working day.

The conditions, set out plainly

Clothing and scheduling first: long sleeves, a brimmed or flapped hat under or with a hard hat, and shade or task rotation around the middle of the day. Sunscreen for the face, ears, neck and hands, kept in a pocket rather than a van.

Why cumulative dose is the point

A holidaymaker might take fourteen days of intense exposure a year. Someone laying kerbs, thatching a roof, crewing a boat or working a farm takes a moderate to high dose most working days from April to September, year after year. The total is not comparable, and it accrues on a predictable set of sites: the face, the ears, the back and sides of the neck, the forearms, and the backs of the hands. There is also an asymmetry that recreational exposure does not produce. Drivers, plant operators and anyone working consistently facing one way accumulate more on one side of the face, one forearm and one side of the neck. It is worth knowing where your own asymmetry is, because that is where to look when you check your skin.

The distinguishing feature of occupational exposure is not intensity, it is repetition. This rail is one day of many. Mechanism only.

Occupational exposure is a scheduling problem, not a product problem. Anything that depends on stopping work to reapply competes with the job and loses. Anything built into what a person is already wearing does not.

A strategy that survives a working day

Build it into what you already wear

A long sleeved work shirt, a collar, and a hat that covers the ears and the back of the neck do more for an outdoor worker than any product, because they require no decisions during the day and do not degrade. Many trades already have long sleeve requirements for other reasons. Where a hard hat is worn, brim attachments and neck flaps designed to fit them exist. Schedule around the middle of the day where the work allows

Not always possible. Where it is, moving the most exposed tasks away from the hours around solar noon is the single most effective control, because it reduces the dose rather than trying to block it.

Keep the sunscreen on your person

In a pocket, not in a van, not in a site office. The distance between a person and their sun protection determines how often it is used far more than any intention does.

Protect the hands deliberately

Hands are washed repeatedly, gloved and ungloved, and abraded all day. They are also the site people apply to last and least. If there is one deliberate addition to a working routine, it is the backs of the hands and the wrists.

Lips and ears

Personal protective equipment cuts both ways

A hard hat shades the top of the head and nothing else. A harness abrades the shoulders. Gloves protect the palms and leave the backs of the wrists exposed. Safety glasses may or may not be ultraviolet rated, and the standard that governs sunglasses is a different one from the standard that governs impact protection. What it certifies

Nothing that a UK or EU regulator has endorsed. In the UK and EU, claims implying that a single application provides protection for a whole day are the kind of claim regulators and advertising codes have long treated as problematic, because they invite people to skip reapplication. The European Commission recommendation underpinning sunscreen labelling advises against claims conveying all day protection.

What it does not

It does not certify that protection survives a working day, and no immersion, abrasion or wear procedure supports it. Anything that rubs, sweats, washes or wipes removes the film regardless of what the front of the pack suggests.

Claims implying whole day protection are exactly the sort of thing the advertising codes exist to police. The CAP and BCAP codes, enforced by the Advertising Standards Authority, require that objective claims be substantiated, and the general framework for cosmetic product claims in retained UK law requires evidence to support them. What is not verifiable

We cannot tell you the dose any particular trade receives, because we have measured nothing and we will not repeat figures we cannot stand behind. We are not going to publish an occupational risk statistic. What we can describe is the mechanism, the pattern of exposure and the controls that reduce it, and point you to the institutions that publish the evidence.

Checking, and when to get something looked at

People who work outdoors have the strongest reason to make skin checking routine, and the least prompting to do it. The NHS publishes guidance on sun safety, and on when to see a GP about a change to the skin. Questions

Is my employer responsible for sun protection at work?

Health and safety obligations at work are a matter for the Health and Safety Executive and for your employer, and we are not the right source for legal advice on it. What we can say is that the controls that work are the ones designed into clothing and scheduling rather than the ones relying on individual memory.

Does a hard hat protect my head from the sun?

It shades the top of the head and nothing else. The ears, the back of the neck and the face remain exposed. Brim and neck flap attachments designed for hard hats address this.

Why do the backs of my hands take the worst of it?

They face upwards for most manual tasks, they are washed repeatedly, they are gloved and ungloved all day, and they are the last place anyone applies sun protection. It is worth making them deliberate rather than incidental.

Can one application in the morning cover a working day?

No. Sweat, washing, gloves and abrasion remove the film long before the end of a shift. Claims implying all day protection are the kind of claim UK and EU guidance advises against for exactly this reason.

I work outdoors year round. Does winter matter?

Ultraviolet in a British winter is low, and the practical concern is much smaller than in summer. The exception is anyone working at altitude or on snow, where reflection and altitude change the picture entirely.
